If this is your first appointment with David (or you need to update your current intake form), please request HIM’s Client Intake Form by email and fill it out - please have it ready for HIM to review with you and collect at your massage appointment.
You’re ready for your massage on your confirmed date, time, and location! You can expect a confirmation call the day prior to your appointment, unless otherwise indicated.

If you would like to use your credit or debit card or Paypal® account…
…payment is required before your appointment time - you may purchase your service here (coming soon). When you let David know before your appointment, he can also bill you by email, where you’ll click on a unique invoice link to Paypal’s® secure website and purchase process using your credit/debit card or Paypal® account.
If you would like to use cash or personal check (with NV ID) for payment…
…please pay immediately after your service is provided.
